Patients across Kent are set to benefit from a groundbreaking digital transformation aimed at improving prehabilitation and elective recovery.
The region has introduced a new Patient Optimisation eForm and its first-ever hospice data feed, providing clinicians with real-time insights to enhance patient care and streamline treatment pathways.
The newly introduced Patient Optimisation eForm is designed to provide a seamless and structured approach to tracking patient health interventions and is a key component of Kent’s elective recovery solution. Developed as part of a broader initiative to improve perioperative care, these digital tools integrate with population health management platforms to support clinicians in risk stratification, triage, and prehabilitation for surgical patients.
By integrating with Kent and Medway Care Record (KMCR), the eForm ensures that clinicians have real-time access to vital patient information, enabling more informed decision-making and efficient care coordination.
One of the key benefits of the eForm is its built-in logic, which ensures data accuracy and consistency. When patients are enrolled in an optimisation service, the relevant fields become mandatory, helping healthcare providers record essential details systematically. The eForm supports multiple services, including smoking cessation, weight loss, lifestyle improvement, and external referrals, reinforcing Kent’s commitment to holistic patient care.
East Kent Hospitals University NHS Foundation Trust (EKHUFT) and Kent Community Health NHS Foundation Trust (KCHFT) are leading the adoption of this innovative tool. Using the elective recovery solution, One You Kent proactively contacts specific patient awaiting elective procedures at EKHUFT, selected as those most likely to benefit most from their services. They offer tailored health interventions to optimise their health before surgery. The new system not only facilitates patient engagement but also allows healthcare teams to track progress and integrate enrolment data into the elective recovery tool.
In addition to the eForm, Kent has launched its first-ever hospice data feed, another breakthrough in improving patient outcomes. This integration enables hospice teams to contribute critical patient data to the KMCR, ensuring that palliative care patients receive coordinated, high-quality support across all care settings.
